BILLING INFORMATION & INSTRUCTIONS
We are pleased to provide you with step by step instructions on submitting claims with the five (5) HMOs. Please CLICK on the links to access further information. Administep can be used to submit claims for AlohaCare, HMSA, and Ohana. United Healthcare & Ohana utilizes their own Portals for Participating Providers only. For Kaiser and non-PAR Providers with the other HMOs, paper claims must be mailed to the HMO's Claims Department using the CMS 1500 Red Claim form that may be purchased from Office Depot or Fisher. HMO rates and billing codes are also provided for your reference. Please call the HMO's Customer Service hotline if you are having issues with your online accounts or your claims.
For Private Pay Clients, you may use our Word format template to send your monthly CCFFH billing invoice. Please refer to the CCFFH Template instructions for assistance in filling out the invoice and how to prorate charges. You are welcome to edit the template or use your own template as you prefer. ALL forms are printable for your convenience.
